We meet with the individual and, when appropriate, their support network to understand goals, preferences, and support needs. -
Person-Centered Support Planning
A personalized support plan is developed based on individual goals, strengths, and desired outcomes. -
Service Implementation
PASA services are delivered by trained support staff in accordance with the approved service plan. -
Ongoing Support & Review
